ATSM and its artists are committed to answering any questions about the tattooing process, the sterilization methods and the equipment used by the shop and the tattoo artists.

- Acknowledge having taken all relevant information concerning the tattoo procedure steps from beginning to end and know that a tattoo is indelible (dermo-pigmentation for life).
- Claim not to be under the influence of drug-alcohol and / or non-prescribed medications.I acknowledge that ATSM and it's staff advises against the use of topical numbing cream and/or any other numbing product. The use of such product and/or any medical consequence is my sole responsibility.
- Warned my tattooist of any health problem that may complicate the tattoo session / tattoo healing and have consulted a doctor for information about my condition with regard to tattooing and my health.
- Recognize that an infection/complication/allergy is always possible and unpredictable. I put the odds on my side by taking good care of my tattoo and respecting the rules of hygiene that applies.
- 4.1 Commits myself to respect all the instructions given to me by my artist concerning the healing process and how to treat my tattoo. Your tattoo artist will provide you with specific aftercare instructions tailored to your tattoo and their preferred methods.
- Declare having freely chosen to get tattooed and have chosen in a thoughtful way the design as well as the artist who will make my tattoo. I have approved my final design and checked spelling( if applicable).I authorize the use of pictures of my tattoo taken by my artist/shop. (if i do not want pictures used on social media i will verbally inform my artist/shop)
- Recognize to be more than 18 years old

COVID19 I have no symptoms of covid-19 and I am not waiting for a test result nor I am in close contact with someone who has covid-19.
I agree to release and forever discharge and hold harmless ATSM and the artists from any and all claims, damages or legal actions arising from or connected in any way with my tattoo or the procedure and conduct used to apply my tattoo. In the event that a problem occurs with my tattoo, I will inform my artist and will consult a medical professional.
